Every detail is thoughtfully curated to create a comfortable, relaxing, and elevated experience from start to finish. 🤍 ROOTEDBYMADS LLC Salon Policies & Service Agreement By booking an appointment, submitting a deposit, receiving services, or making payment, you acknowledge that you have read, understood, and agree to all policies outlined below. These policies are available prior to booking and are considered a binding agreement between the client and RootedbyMads LLC (“Stylist”). ⸻ 1. APPOINTMENT RESERVATIONS & DEPOSITS A non-refundable deposit is required to reserve appointment time. Deposits are applied toward the final service total unless otherwise stated. By booking an appointment, the client acknowledges that the Stylist reserves dedicated time specifically for that appointment and may decline other booking opportunities during that reserved time. Deposits are non-refundable under all circumstances, including but not limited to: * Cancellation by the client * Rescheduling by the client * Change of mind regarding services * Failure to appear for the appointment * Dissatisfaction with the final result when services were performed as agreed upon during consultation ⸻ 2. CANCELLATION, RESCHEDULING & NO-SHOW POLICY A minimum of 72 hours’ notice is required to cancel or reschedule an appointment. Appointments canceled, rescheduled, or modified within 72 hours of the scheduled appointment time may be subject to a cancellation fee of up to 100% of the reserved service amount. Clients who fail to appear for a scheduled appointment without notice (“No-Show”) may be charged up to 100% of the reserved service amount and may be required to prepay future appointments. Repeated cancellations, no-shows, excessive rescheduling, or failure to comply with salon policies may result in refusal of future booking privileges. ⸻ 3. NO REFUND POLICY All sales are final. Due to the customized nature of salon services, professional expertise provided, and products utilized during the service, RootedbyMads LLC does not offer refunds for any completed services, deposits, retail products that have been opened or used, gift certificates, or prepaid services. Payment for services acknowledges acceptance of the completed service. ⸻ 4. SERVICE ADJUSTMENT POLICY Client satisfaction is important. If a client believes an adjustment is necessary, they must notify the Stylist within seven (7) calendar days of the original appointment. Adjustment requests: * Must be submitted within 7 days * Must relate to the original consultation and agreed-upon service goal * Must allow the Stylist an opportunity to inspect and assess the hair in person Adjustments are offered solely at the Stylist’s discretion. The following are not considered adjustments: * A change of mind regarding color, length, tone, placement, or style * Requests for a different look than originally agreed upon * Services performed by another stylist after the appointment * Damage caused by improper home care, swimming, heat styling, medications, or undisclosed chemical history No refunds will be issued in lieu of an adjustment. ⸻ 5. CLIENT CONSULTATION & DISCLOSURE REQUIREMENT Clients are solely responsible for providing complete, accurate, and truthful information regarding their hair history and condition. Clients must disclose all relevant information including, but not limited to: * Previous color services * Box dye usage * Lightening or bleaching history * Keratin treatments * Relaxers, perms, or chemical straightening services * Extensions * Hair loss or scalp conditions * Allergies or sensitivities * Medications that may affect hair processing * Recent medical treatments that may affect hair integrity Failure to disclose relevant information may negatively impact results and may increase the risk of damage. RootedbyMads LLC shall not be held responsible for adverse outcomes, breakage, unexpected color results, compromised hair integrity, allergic reactions, or other issues resulting from inaccurate, incomplete, or withheld information. ⸻ 6. CHEMICAL SERVICE DISCLAIMER All chemical services involve inherent risks. The client understands and acknowledges that results cannot be guaranteed due to variables outside the Stylist’s control, including but not limited to: * Previous color history * Box dye * Prior chemical services * Medication usage * Water quality * Hair condition and integrity * Home care practices * Hormonal changes * Environmental factors The client understands that: * Hair may not lift or process as expected * Multiple appointments may be required to achieve desired results * Hair integrity may limit what can safely be achieved during a single appointment The Stylist reserves the right to modify the service plan if necessary to protect the health and integrity of the client’s hair. ⸻ 7. RIGHT TO REFUSE, MODIFY, OR DISCONTINUE SERVICE RootedbyMads LLC reserves the right to refuse, modify, postpone, or discontinue services at any time and for any lawful reason, including but not limited to: * Hair integrity concerns * Undisclosed chemical history * Medical or scalp concerns * Unsafe working conditions * Inappropriate, threatening, disrespectful, or disruptive behavior * Unrealistic expectations * Requests that may compromise the health of the client’s hair If a service is refused or discontinued due to the circumstances listed above, deposits may be forfeited at the Stylist’s discretion. ⸻ 8. MINOR CLIENTS Clients under the age of eighteen (18) must have permission from a parent or legal guardian to receive services. By booking services for a minor, the parent or legal guardian acknowledges and accepts all risks associated with the services performed. RootedbyMads LLC shall not be liable for disputes regarding parental consent. ⸻ 9. PHOTOGRAPHY, VIDEO & MARKETING RELEASE The client grants RootedbyMads LLC permission to photograph and/or record completed services. Such images and recordings may be used for: * Social media * Marketing materials * Advertising * Educational content * Professional portfolio purposes No financial compensation shall be provided for the use of such images. Clients wishing to opt out must submit a written request before the appointment begins. ⸻ 10. INDEPENDENT BUSINESS DISCLOSURE RootedbyMads LLC operates as an independent business within a salon environment. The salon facility, property owner, salon management, and any affiliated parties are not responsible for: * Services performed * Pricing * Policies * Scheduling * Outcomes * Client disputes All matters relating to services provided by RootedbyMads LLC must be addressed directly with the Stylist. ⸻ 11. PRICING POLICY All prices listed online, verbally quoted, or displayed in marketing materials are estimates unless otherwise specified. Final pricing is determined by factors including: * Hair length * Hair density * Product usage * Time required * Corrective work * Desired outcome * Additional services requested The final service price may differ from the original estimate if additional work or product is required. Any promotions, discounts, package pricing, referral rewards, or special accommodations must be disclosed and confirmed at the time of booking. Discounts cannot be applied after services have been completed. ⸻ 12. LIMITATION OF LIABILITY To the fullest extent permitted by law, RootedbyMads LLC, Madison King, and any affiliated parties shall not be liable for: * Hair breakage * Color fading * Allergic reactions * Chemical sensitivities * Unexpected processing results * Loss of personal property * Dissatisfaction resulting from unrealistic expectations * Outcomes affected by undisclosed information * Damage resulting from failure to follow aftercare instructions The client voluntarily assumes all risks associated with salon services. ⸻ 13. AGREEMENT & ACCEPTANCE By booking an appointment, paying a deposit, receiving services, or submitting payment, the client confirms that they: ✓ Have read and understood these policies ✓ Had the opportunity to ask questions regarding these policies ✓ Voluntarily agree to all terms and conditions ✓ Accept responsibility for providing accurate information regarding their hair history and condition ✓ Consent to services being performed under these terms ⸻ 14. CONTINUING AGREEMENT This Service Agreement and all salon policies shall remain in effect for all appointments, services, consultations, transactions, and interactions with RootedbyMads LLC unless otherwise updated in writing. By booking an appointment, submitting a deposit, receiving services, or making payment, the client acknowledges that this agreement applies to all past, present, and future services provided by RootedbyMads LLC and does not require re-execution for each appointment. The client further acknowledges and accepts responsibility for any appointments scheduled under their name, including appointments booked for minors, family members, dependents, or any other individual. The person making the appointment agrees to ensure all parties are aware of and bound by these policies and assumes full responsibility for compliance with this agreement. RootedbyMads LLC reserves the right to amend, modify, or update these policies at any time without prior notice. Continued booking, payment, deposits, consultations, or receipt of services constitutes acceptance of the most current version of these policies. ⸻ ROOTEDBYMADS LLC Madison King, Owner & Licensed Cosmetologist By proceeding with any booking, consultation, deposit, payment, or service, the client acknowledges that they have read, understood, and agreed to all terms outlined in this Service Agreement and Salon Policy Contract. ✨ This document shall remain binding for all current and future services unless otherwise superseded in writing by RootedbyMads LLC.Business Hours

I kindly ask for at least 72 hours’ notice for any appointment changes or cancellations. This allows me the opportunity to accommodate other guests on my waitlist.
NO refund policy.
By booking any and all services, through stylist or online you are agreeing to all terms and conditions.
Cancellations made with less than 72 hours’ notice will be subject to a fee and may require a deposit to re-book future services.
Multiple late cancellations or no-shows may result in your online booking access being blocked and services requiring prepayment.
I understand that emergencies happen, and I’ll always do my best to work with you. Thank you for respecting my time as much as I respect yours.
